-Work with Community Navigators and Support Coordinators on Person-Centered Plans resubmissions and corrections.
-Provide step-by-step guidance on conducting effective PCP meetings to new Community Navigators and Support Coordinators once they have completed their three-week new hire training.
-Conduct shadowing sessions, allowing hires to observe while facilitating and entering Person-Centered Plan meetings.
-Work with Individuals with Developmental Disabilities on developing Person-Centered Plans outlining their needs, wants, and goals.
-Track service recipient goal progression.
-Maintain at least monthly communication with service recipients and/or their team and maintain accurate documentation.
-Assist service recipients with accessing community-based services and resources.
-Provide guidance on disability-related benefits such as SSDI, SSI, Medicaid, and vocational rehabilitation services
